como coloco esse aviso em cima do ticket ?

qual o nome do arquivo tpl que devo editar.

só uma duvida pra eu colocar algum arquivo css pra rodar dentro do supportticketsubmit-steptwo.tpl eu tenho que colocar 

{include file="style.css"}

 

exemplo assim em em cima como funciona ?
